Thank you for your interest in piloting the Well Aware app! To receive your free test kit, please fill out the secure Google Form below. Your information will only be shared with our lead researchers.

After you complete the interest form, we'll get in touch with you and send a test kit your way. Once you receive the kit, you'll need to download the Well Aware app from the Apple App Store or the Google Play Store. The app will read your test and provide you with accurate results. After you complete your kit, we'll ask you a few basic questions about the app, the test kit, and the instructions to identify ways to improve Well Aware.
